Creating a new connection problem

I had a broadband connection configured fine and working.  Once when I
rebooted by my system the desktop icon was not working.  I went to my
NetworkConnections panel and the broadband connection was not there.
When I tried creating a new one I got as far as the 'how do you want to
connect to the internet' page. The first 2 options are grayed out. The
only option available is 'connect using a broadband connection that is
always on'.  The option I need, which is: 'Connect using a broadband
connection that requires a user name & password' is grayed out.

Anyone have any ideas?
